Males have a yellow , while females have a black head. The pronotum is black with a yellow margin and anterolateral angle. Females have black with a basal spot and an apical spot. Males possess an additional humeral spot on the elytra, giving them three spots per elytron versus two in females. flavifrons differs by having an additional marginal spot on the elytron.

Eastern and central North America. The nominate B. q. quadripunctata ranges from Massachusetts and New York south to Virginia and Tennessee, west to Iowa and Kansas. Subspecies B. q. flavifrons occurs in the southeastern coastal plain: Alabama, Florida, Georgia, North Carolina, and South Carolina.
